By completing this online registration, I understand and do hereby affirm the following:
1. I am a legitimate alumnus/alumna of Parañaque Science High School (ParSHS) [or I have attended ParSHS for at least one year];
2. I will adhere to the ParSHSAA Core Values which are: Patriotism, Accountability, Respect, Service, Honesty, and Sociability;
3. I am aware that membership to ParSHSAA is voluntary and that the Association emphasizes the importance of the spirit of volunteerism;
4. I will do my best to get involved and share meaningful contribution/s for the achievement of ParSHSAA’s vision, mission, and strategic goals; and
5. I have the responsibility to report to proper authorities if ever I observe any wrongdoing, illegal or fraudulent activities (e.g. hazing, abusive practices, or any form of behaviors that are direct transgressions of the Association’s core values, constitution & by-laws, and official policies) will occur or have occurred within the purview of ParSHSAA.
I really miss ParSci! Thanks to the people who made possible of the website and of course the Alumni Association! ParSci always rocks, yeah!

As you can see, this is the 2nd form that i have created using formlogix, but don’t worry i’ll be making another one. I’ll keep you guys posted. Thanks for your patience.

Medyo nagkakaroon kasi ng problema tungkol sa limited database. We’re still testing some other options para hindi na laging “maximum row limit” ang nakikita niyo.
